Can anyone tell me how perforation errors like this occur. Bottom of the coil set of the middle stamp, How a coil set gets just one ?? But then also how the green line running down the middle and right stamp gets smeared. something just looks off about it. Just curious

The "extra hole" at the bottom of the middle stamp in *not* any part of the production process nor any error, but rather damage at some point after its sale to the public.
