It has a double row of perf. at the top?...How did they get there?

Please, crop you picture.

There is a second stamp, part stamp, or margin stuck to the back. I am not familiar with the make up of US coils. In many countries, they had paste-up coils where coil joins can result in something similar.

No, not a paste-up coil, nor is it a double row of perfs. It simply looks like the perforations didn't cut through all the way, and the partially punched chad is still attached to the stamp.

The unpunched perfs are also sometimes called "blind perfs" when the perforator just lightly dents the paper and doesn't cut through.

In Florida terminology, you have "hanging chads".

Here is a block of the same stamp with the middle row of perforation chads mostly remaining in place. If it were separarted into singles, the lower two stamps would look just like yours, even to the point of the perforation row being high enough to show the extreme bottom edge of the stamp above it.
